Case Management

Our Case Management program is a collaborative process the client and Case Management. It includes: client assessment, tasks planning, facilitation and joint discussions to meet the health, legal and social service needs of our community. Our case managers are dedicated staff members and work with individuals and families to apply for social services such as food stamps, health insurance, housing, Medicare, and social security. They also provide referral assistance and help identify local resources to aide clients.

Our case manager interviews and evaluates each client to assess their needs and to develop a proper action plan. We provide ongoing follow-ups, referrals to social service and health agencies, facilitate small workshops, and provide informational materials for educational and guidance purposes. We believe that engaging clients and encouraging them to take an active part in managing their affairs.

It is important that our clients see us as “guides”; not permanent errand runners for their personal and business affairs. This is crucial to their empowerment, independence and success.
